Your Support Coordinator works alongside youto connect with providers, manage your funding, navigate services, and makesure your plan is genuinely working for you.

Look for "Support Coordination" or "Specialist Support Coordination" under Capacity Building in your plan. If you’re unsure, send us a copy and we’ll check for you.
Yes. You have the right to choose your Support Coordinator and change at any time. We’ll help make the transition smooth.
It depends on your plan and what’s happening, but most participants hear from us at least monthly, with more frequent contact during plan reviews or when supports are changing.
Specialist Support Coordination is for participants with more complex needs — for example, multiple services, high-risk situations, or significant barriers. We can help you work out which one fits.
